What is Immanuel Pathways PACE®?

Immanuel Pathways serves seniors through PACE® (Program of All-inclusive Care for the Elderly), an innovative, national model that offers a complete health plan for individuals with chronic health care needs who wish to remain in their homes. 

The PACE movement is one that began in the 1970s, when medical professionals were looking for home-based programs to help their patients. The first center was opened in San Francisco with 10 more by 1990. A national PACE program was developed at that time to focus on community seniors. Today, there are more than 270 PACE centers in the US. Immanuel operates the only center in Nebraska and two of three centers in Iowa. 

What services are offered through Immanuel Pathways? 

Immanuel Pathways PACE centers are hubs for seniors to receive medical appointments, life enrichment, social opportunities, wellness activities and more. With transportation to and from the center, as well as meals and snacks provided, this program is invaluable to community seniors and their families. Services at Immanuel Pathways include, but are not limited to: 

  • Primary care
  • Prescription drugs
  • Physical and occupational therapy
  • Dentistry
  • Social services
  • Nutritional counseling
  • Recreation and life enrichment
  • Transportation


What are the benefits of Immanuel Pathways? 

One of the biggest benefits of Immanuel Pathways PACE center is the interdisciplinary team. All doctors, therapists, social workers, even dentists work together, meaning there’s no phone calls, no sending over records, no hassle in making sure everyone is on the same page regarding care. Care plans are made collaboratively with the goal of whole-person wellness. 

This year, during the ups and downs of COVID-19, the interdisciplinary team really came together to ensure participants were well cared for. Phone check-ins, home visits, even delivered care packages with masks, treats and puzzles were immediately implemented. This was great for participants, but also for families who received the peace of mind their senior loved ones were looked after and didn’t have to quickly arrange for alternative home services. 

And during more typical years, participants and families love Immanuel Pathways. Through a third-party survey in 2017, participants rated a 97% overall satisfaction of services. And participants reported: 

  • Better self‐rated health status
  • Less pain interfering with normal daily functioning 
  • Less likelihood of depression 
  • Better management of health care 

Plus, for those eligible for Medicare and Medicare, Immanuel Pathways costs are provided at no charge. 

Who is Immanuel Pathways Right For? 

As part of a nationwide program, there are eligibility requirements for PACE services at Immanuel Pathways. Participants must: 

  • Be 55 years or older
  • Live in an Immanuel Pathways service area
  • Assessed to meet nursing facility level of care
  • Able to live safely at home with the support of Immanuel Pathways

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
