Immanuel Pathways
Central Iowa
Eligibility
Our Pathways center includes preventative care and services covered by Medicare and Medicaid as authorized by our interdisciplinary team. It combines to create an environment where nursing home qualifying seniors can continue to live independently at home.
Pathways participants must meet certain eligibility requirements to participate in the program:
- Aged 55 or older
- Assessed to meet nursing facility level of care
- Able to live safely at home with the support of Immanuel Pathways
- Lives in our Immanuel Pathways Central Iowa service area serving Boone, Dallas, Jasper, Madison, Marion, Marshall, Polk, Story and Warren counties.

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
