Eligibility
For participants and families, there’s no greater peace of mind than knowing there’s an interdisciplinary team working together to ensure you can live independently at home for as long as possible. That’s the benefit of Immanuel Pathways centers. We cover all preventative care as well as our team’s services covered by Medicare and Medicaid. To participate, individuals must meet the following eligibility requirements:
- Aged 55 or older
- Assessed to meet nursing facility level of care
- Able to live safely at home with the support of Immanuel Pathways
- Lives in our Immanuel Pathways Eastern Nebraska service area serving select zip codes of Douglas, Sarpy, Cass, Dodge, Saunders and Washington counties.

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
