Immanuel Pathways
Southwest Iowa
Care & Services
Seniors who are eligible receive an interdisciplinary team that includes specialized physicians, therapists, nutritionists, social services professionals and more, dedicated to helping maintain health and wellness at home.
Transportation to and from the Pathways center is included. A meal and snacks are provided. Social engagement and activities are offered. All services are provided to participants from dedicated, expert staff who have made it their mission to serve seniors.

- Transportation to the PACE center and medical appointments

- Primary care
- Prescription drugs
- Emergency services
- Physical therapy
- Occupational therapy
- Dentistry
- Social services
- Nutritional counseling
- Hospital stays, if needed
- Nursing home stays, if needed
PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
