Imagine all your health care needs are taken care of, including transportation to and from appointments and access to enriching social opportunities. That’s life at Immanuel Pathways. Eligible seniors receive support from our interdisciplinary team of physicians, nutritionists, therapists, social service professionals, dentists and more.
Between appointments, participants enjoy a meal and snacks and can participate in social engagement activities. This revolutionary approach to healthy aging enables nursing home eligible individuals to continue living independently with access to all the support they need for whole-person health and wellness.

- Nursing home stays, if needed
PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
