Immanuel Pathways
Southwest Iowa
Eligibility
Immanuel Pathways enables nursing home qualifying seniors to continue living in their homes for as long as possible. We encourage and cover preventative care, as well as all the services covered by Medicare and Medicaid as authorized by our interdisciplinary team. Pathways participants must meet certain eligibility requirements to participate in the program:
- Aged 55 or older
- Assessed to meet nursing facility level of care
- Able to live safely at home with the support of Immanuel Pathways
- Lives in our Immanuel Pathways Southwest Iowa service area serving Mills, Harrison and Pottawattamie counties.

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
